Submit your ERC lead and schedule the appointment all at once by filling out the form below.

Helpful Hints:

  • The ERC wage qualification period is 1/1/2021 - 9/30/21

  • A minimum of five W-2 employees or more, not counting owners or family members on payroll.

  • Submit ONE lead for multiple entity owners. You will always be credited with all associated entities.

  • If rescheduling, do not resubmit the lead, simply reschedule the meeting HERE.

  • Use the Portal to submit notes on your lead if needed.

Examples of Modifications to Business Operations:

  • Limiting occupancy to provide for social distancing

  • Requiring services to be performed only on an appointment basis (if business previously offered walk-in service)

  • Elimination of waiting rooms or gatherings

  • Reduction of capacity in a dining room or waiting room

  • Change in service format (Ex: restrictions on buffet or self-serve dining; walk-in vs. curbside

  • Reduced operating hours

  • Reduction in employee hours worked
